2026-2027 Open Enrollment CANCELLATION

PLEASE READ IMPORTANT INFORMATION BELOW:

The Open Enrollment Cancellation Request Form should ONLY be completed if you NO LONGER want your child open enrolled for the 2026-2027 school year.

One form MUST be completed for each child requesting the Open Enrollment Cancellation.

IMPORTANT: If you wish to CHANGE your child's Open Enrollment placement (e.g. you would like to request a different school that is NOT your home school) DO NOT COMPLETE THIS FORM!

Completion of the Open Enrollment Cancellation Request guarantees your child's return to his/her Home School. You will NOT receive an approval notification for your request.
